I've been trying to do this for over a year.  Our primary focus is web hosting
both in a UNIX environment and NT environment (depending on customer
needs/requirements).  My long-standing architecture goal was to provide a
robust file server environment within AFS for all websites (and other 
projects) regardless of platform.  It has worked very well in the UNIX
environment but we ran into exactly the same problems you did with NT/IIS
accessing AFS volumes.  I have done quite a bit of testing including some
experimentation to see if there was any difference using Samba.

The bottom line is that, to date, I have not been able to get it to work.  I
opened up a problem ticket with Transarc on this issue over a year ago and
occasionally check back on progress.  Transarc has been able to duplicate the
problem so I assume they are working on a solution but I still haven't heard of
a fix.

It was interesting to note that IIS/NT accessing a Samba share that pointed to
an AFS volume produced the same results (no matter how open I made the Samba
end).  A Samba share that pointed to a local piece of the UNIX file system (not
an AFS volume) worked just fine.  

I would DEARLY love to hear of a solution if anyone has one.
