We had this happen to us.  It wasn't fun.

These symptoms were present when we tried to use "fs setserverprefs" to
more intelligently choose server machines since we have many subnets in
our cell.  A bug was discovered and summarily fixed involving a cache
manager field not being initilized properly.

You can avoid the bug by accessing files on the servers prior to
referencing them in a "fs setserverprefs" command or you can contact
your friendly AFS Technical Support person and request a patch.

Marybeth Schultz was our TS person and would be familiar with problem.
Have your TS person call my TS person. ;-)
