There is a known AIX problem that could cause a server to lose a partition.
The machine will panic with a stack trace similar to:

> .vmvcs ()
> .vmfcopyin ()
> .vm_move ()
> .writei ()
> .xix_rdwr ()
> .vno_rw ()
> .rwuio ()
> .rdwr ()
> .kwritev ()

One of our IBM site contacts reports:
> I got my branch office to admit that this is an AIX, not an AFS
> problem. They (AIX support) told me to install U417683.
>
> The essential problems solved by this fix are
> U417683 IX25070 bos : Serialization between freeiblk and allociblk
> Not Correct.
>
> You might have had this one already.
> U417683 IX35007 bos : SYSTEM CRASHED AT VMVCS.
PLEASE verify that you have this fix installed.
