Actually, this is very platform specific, and on many platforms it is
not even AFS which is doing the mmap.  For my Linux-AFS port, I have a
read-only mmap, such that you can mmap a file in read-only mode only.
This is accomplished by reading the page into memory when a page-fault
occurs.

I don't know what other platforms do, but this tends to be fairly good
performance (IMHO).
