I have just finished mopping the kitchen floor, gosh us techos and
developers are messy folk. Maybe its because we are human? :^)

Not any more :-) 1.5d on the production machine, a full download from your
website to make sure we were tottaly current :-) Remember it was this
install that found the "didn't make the queue directory on a new install" bug.
Here is the log entry tho':

[inFusionEngine 2.0.1] started
I think a version got lost somewhere :-)
Your question made me notice a HUGE clue tho'. All of the entries that did
not follow on with logging had the above log line missing from the
beginning of the list of items. The "1st" items in a bad one were:
19/01/2001 09:51:15 Skip HELO template enabled
19/01/2001 09:51:15 Skip MAIL template enabled
The "===" lines seem intermittent, are they meant to be before and after
the server start-up entries? If so that was broken as well.
