What does "Connection Refused" mean?  Is it specific to being refused from
my domain, or is it that there's no mail server there?  I'm trying to decide
how to treat these addresses.

Do ALL messages that go through PostFail  also go through ReportPostStatus
(if Report Failures is checked)?  If so, then they'd have the ident token,
which would make them easier to process.  If not, what types call PostFail
that doesn't get picked up by ReportPostStatus?
