I would consider anything with a blank To or From as Spam. Most emails w/o a subject
(or wth a subject that contains 10 or so spaces in a row) are also Spam.
I have a bunch of other things in our servers here that trigger email as Spam:
1 - we use 3 RBL servers (spamcop, njabl and spamhaus)
2 - mail that has a helo that is the IP address of our mail server or the rdns of our
server is spam. For example:
HELO 64.69.103.207
HELO rabbitt.hoot.com
Spam still comes through but our servers block around 95%
