My first question was the From address: "<[EMAIL PROTECTED]>"

Is someone is spoofing your address? Or do you expect to see this in mailing lists?

I could not find the send to addresses in any of my mailing lists.

Could one of my users send mail to a list where the send to addresses are in cc/bcc and spoof the return address? How would they find "<[EMAIL PROTECTED]>"?
