Dave,

To answer your third question, to find out why your connection was refused,
you can turn on protocol logging in the configurator. It's located on the
POST tab. Once you turn it on you'll have to send the mail again for it to
log the extra details.

However, you don't want to leave it on, because your log files shall become
quite large. So, you should turn it off once you have the information you
need.
