There are a few, though we really ought to try to get a reasonably
complete list and this is just ad-hoc:
1. not wanting to break browsers that prefer IPv6 but don't backoff and
try IPv4
i.e., not wanting to look black-holed in those cases
2. lack of network management in IPv4
one of the paradoxes, at least as net admin folk tell me,
is they need IPv4 to monitor and administrate an IPv6 net
3. a (fairly accurate) belief that it's not worth the cost
