For CoD2 on Vista and XP, you will have to run it in 640x480 resolution, bilinear graphics, everything on low (though you can change the graphics in the "Custom" area to extreme and still get around 26-60 fps while in-game, usually ~25 fps in battles, the corpse rate I have tested and does effect the fps as well but will run at ~25fps even on extreme).
Have you tried the 1.3 patch?
