I was on the right track with the getty, turns out that Apple enable the Xserve's serial port as a terminal line by default!
On the Xserve open: /System/Library/StartupItems/SerialTerminalSupport
About line 72 is: ENABLE_SERIAL_TERMINAL=$TRUE
Change this to: ENABLE_SERIAL_TERMINAL=$FALSE
Re-init the system, and there should be no getty and InterMapper will get to the modem just fine.
