Custom icons with intermapperd?

Working up the 4.0 release for upgrading our production installation, but I've run into an issue with custom icons not coming through intermapperd maps.

I've put our icon library files everywhere I can think of, restarted everything several times - yet still no custom icons.

I'm probably missing something obvious, but I'm out of ideas at this point.
