We've looked into this issue and it looks like a bug in intermapperd 4.0. intermapperd does not load the icons stored in resource files in the "Custom Icons" folder. This will be fixed for 4.0.1.

InterMapper 4.0 now also supports PNG and JPEG formats for custom color icons. PNG and JPEG are the only custom icons that intermapperd will currently load, and they must have the correct file extension: .png or .jpg. There are limitations to the support for PNG however -- they must be non-interlaced images with a palette (ie indexed color table).
