Your right. The CatOS (Hybrid mode 65xx, 55xx, modular Cats...) doesn't translate to mod/port. I've tried all the combinations to get it to report properly. I'm guessing it's more of a MIB defined issue, then a IM. The 2950s, 3550s, 35xx/29xxXL Cats are IOS (router) based and do report properly. My work around is to do a "show port ifindex" and then change the label manually.
