Can you monitor the Xserve with Server Monitor? If not, then there may be a configuration error on the Xserve.
You may need to install the Admin Tools that came with the Xserve. If that still doesn't work, and Server Monitor says "Failed to contact server" in the log, then try adding the following line to /etc/watchdog.conf and restarting hwmond:

 hwmond:respawn:/usr/sbin/hwmond # Hardware Monitor daemon

The /usr/sbin/hwmond process is a daemon that monitors the hardware periodically (try 'man hwmond'). hwmond will currently only run on XServe hardware. It has been suggested that this situation could arise when installing OS X Server from another (non-XServe) machine. In this case, it may be that the pertinent line in /etc/watchdog.conf gets removed because the installer is not being run on an XServe.
