Re matching software / firmware versions: the requirement is that major
version of software <= major version of firmware. That's why any 3.x would
work with any 3.x etc.
IOIOManager does not work on all Android devices. Specifically, ones that
have potentially diverged significantly from the "pure" Android sources.
This is the main reason why I moved away from this approach with the
IOIO-OTG. If you are certain that you are using IOIO Manager correctly
(double-check with the Wiki page), I would try a different Android for the
upgrade process. Note that any Android >= 4.1 is known *not* to work, so
you'd need to find an older one.
As an alternative, if you have a second IOIO or a PIC programmer, you can
flash a new image on the PIC containing the bootloader with a later app
firmware.
