First, a recommendation: upgrade your application firmware to the latest.
You're using an ancient version.
Second, the fact that you're getting the accessory pop-up means that
everything is OK with the IOIO and your phone and you probably have some
problem with the application. Try the pre-build HelloIOIO.apk from the
latest software bundle. If it works (probably will), you have a problem
with the way you're building the application. Have a look at the eclipse
troubleshooting page on the wiki and report back if you still have
questions.
