There was recently a similar issue posted with no clear resolution yet. Is
it possible that your hacked USB cable is somehow shorting the USBID pin,
forcing the IOIO into host mode? Can you try a standard cable?
Also, which app did you flash? Are you confident that flashing succeeded?
