This is you inadvertently doing it most likely.   That is what happens to me.  
You grab the phone to put it back in your pocket, for example, and you 
accidentally touch the volume buttons.  Or in your pocket, the buttons get 
pushed when you bend over and something else presses against the phone, or it 
is in a position where the walking motion causes the phone to push against your 
leg or the outer part of the pants pocket material and push it.  (Besides 
occasionally on the phone, I also have this problem with my truck, and to a 
lesser extent, the car key with the clicker attached, either unlocking the car, 
or more often, setting off the panic alarm when I bend over or things shift in 
my pocket)

The worst, for me, is where the "mute ringer" button accidentally gets toggled.
