this PR is removing anything related to page caching. In its replacement
there will be a inUse page on PagingStore, that will have the current reading
page for each cursor.
I could have an option to remove all caches and always read from file, but
that would have a drastic semantic change.
With this change, cursors that are sharing the same reading point will seve
a lot of IO.
