@turboFei, `FormParam` is case-sensitive. Parameters `QueryParam` and
`FormParam` are case-sensitive in the JAX-RS specification. Meanwhile, the
parameters in `monitoring.md` document is in lowcase. Therefore, do we need to
support case insensitive parameters?
