You should just put your name in the Signed by and print name fields, put
nothing in the title field, and either put nothing or put "himself" in the
Representing fields.
In the exhibit A, you should list the contributions, typically by putting the
files names.
If you plan to contribute more code, then the next steps will depend on how the
additional contributions will be done.
If they are completely created outside of Apache and later imported into Apache
(for example because they already exist or you plan to develop them for a
non-apache project and only later contribute them), then you will need to fill
another software grant (one for each contribution).
If on the other hand they are created specifically for Apache and if you plan
to contribute regularly (which we hope, as Apache is mainly devoted into
extending communities), then it would be worth to sign an Individual
Contributor License Agreement (ICLA), which is described in the same web page I
cited, in the section jest before the software grant section.
The rationale is that a Software Grant is specific to one piece of code that is
developed outside of Apache, and imported into the foundation, whereas a
Contributor License Agreement is specific to one person and covers all work
this person will do later on in the scope of the various Apache Projects.
