OK. I think everything is all set now. The 2.1.x/2.0.x branches no longer
create a new interceptor per request. (that was just nuts and unneeded).
I've updated trunk to have the MessageModeOutInterceptor call the
SAAJOutInterceptor to get the factory it has.
