Summary: Fix amortization mapping during COB of a re-opened loan

# A loan was disbursed, and a Capitalized Income or Buydown Fee was created
# The loan was fully paid off, either by a (payout) refund or a repayment.
After this the loan status transition to {{CLOSED}} (600).
# Later the transaction (refund or repayment) is reversed, re-opening the
loan, and loan status transition back to {{ACTIVE}} (300).
# In the following COB after reversal, Capitalized Income Amortization or
Buydown Fee Amortization was created in the {{m_loan_transaction}} table, but
they don't have a mapping in the {{m_loan_amortization_allocation_mapping}}
table. This cause the Transaction Summary Report to be incorrect.
