{{CallContext.isArgumentNull()}} returns true for both {{DEFAULT}} placeholders
and explicit {{NULL}} values. Add {{isArgumentDefault(int pos)}} to distinguish
them while preserving the existing {{isArgumentNull()}} semantics.
