Also, when the typical username/password login form initially appears, the 
username field doesn't always received focus. Whether focus is received by the 
username field seems random, though for me it almost always remains unfocused.

The password field is consistently focused if an invalid username/password 
combination is provided, and the username field is consistently focused if a 
blank form is submitted, but focus on initial page load is inconsistent.
