We're using an internal version 3.0.4, which is based on 3.0.0. This only 
happened a couple times in our environment. In one instance the previous dead 
meta server node was removed from the cluster automatically (perhaps due to 
maintenance). But the meta znode was never updated. I'm working on another 
patch to make sure meta state and its znode gets updated as soon as the 
server's ephemera znode expires. That should reduce the chance of getting into 
this state. But as [~wchevreuil] mentioned, this could happen in case of 
HBASE-21843 or other extreme cases. Having those logs could help debug the 
issue.
