Correctness wise this looks pretty good and it does remove a lot of goofy code.
Please have a careful look, this will impact Increment/Append performance as
each increment will no longer remove the old KVs from the memstore (if it is
still found in the memstore).
But it will make it correct w.r.t. version and TTL handling and remove any
special status from Increment and Append.
