351337e adds support for cancelling an RPC while it is mid-transmission.

I think the only remaining improvement we might make here is a cancellation 
that actually gets sent to the remote and propagated into the server's 
RpcContext, indicating that the client has given up and no longer wants the RPC 
to be processed. That seems like a significant new chunk of work, so let's do 
it in a separate JIRA if/when we find the need for it?
