   Thanks for the review. This is addressed in the latest commit (CI green).
   
   The CSV format-table guard now also rejects a non-trailing ADD COLUMN — 
including an ADD followed by a position move that shifts existing physical 
columns — symmetric with the existing non-trailing DROP guard. The `(id, extra, 
label)` positional mis-decode no longer happens: the mutation is refused with a 
clear error instead of producing a silently wrong permissive read.
   
   Added a test asserting that a position-shifting ADD is rejected while a 
trailing ADD is still accepted, alongside the existing DROP case.
