If -direct is mandatory, then there's no purpose in having a -direct parameter.
Parameters exist because users have choices – if they don't have a choice then
why have a parameter?
Of course, we shouldn't fail if we get a -direct option because users may have
legacy scripts written assuming it's there, but I don't think we should require
it, and it should have no effect if it's there – we'll do the direct path
either way.
Also good to retire the PhoenixIndexImportMapper that does the bulk loads,
since it won't be used anymore.
