scroll/scan is not supported in ES 5.x
Scroll/scan was deprecated in 2.x, and has been removed in 5.x.
The replacement is to sort by _doc order, which is optimised.
The elastic.scan method can be dropped.
Callers need to allow for results from the first ES query.
