Facsimile of Merge Error

I recently upgraded to OpenOffice 3.3, build 9567. I have been using Open
Office Writer for the past 5 years to merge data in various documents with
great success. For the past two years I have been merging barcode and other
data from a Calc Spreadsheet into a document that is used as a questionnaire to
collect survey data. In fact, I just printed a set of approximately 70
questionnaires in the last two weeks without error using OO 3.2. This is the
first time I have merged documents since upgrading from 3.2 to 3.3. 

Please view the PDF attachment that is a facsimile of a printed questionnaire.
Notice that the first 2 pages print without error; the Template Number and
Survey Serial Number have been correctly fed into their respective fields. In
pages 3,4,5, and 6 the Template Number and Survey Serial Number have not been
fed to their respective fields. This is the case for every set that I have
printed and in several sets there has been the inclusion of an extra blank
page.

It may be that I need to make additional changes to my document to meet new
format requirements. If this is the case, please let me know what changes are
necessary. I have checked the fields to make sure that the primary document
will accept the merge fields on each and every page using View, Data Source.
