Updated patch with a couple of unit tests. Couldn't add a test for
PipelinedShuffle without making some classes non-final so haven't added that.

Regarding the Fetchers, I don't think the exception needs to be changed there.
The Fetchers are controlled by Shuffle or ShuffleManager - both of which
control the Fetcher threads. They expose a different mechanism to the Reader /
Input (Shuffle.waitForInput and ShuffleManager.getNextInput) to fetch Inputs -
and this is modified for the different exception.
