In the latest version of OOo, 2.4.0, the help and the Change Icon prompt have
both been changed to state that the icon size must be 16 x 16, so this is no
longer an issue. What I was asking for was done.

However, if you really want a step by step, please send me another email and I
will try to provide it. I don't have 2.3.0 loaded anymore, so it will be harder
and I will have to do it from memory, but you can't reproduce this in 2.4.0
because it won't happen.
