         For copying of data at the level that you need - you should 
be skipping the high level objects and going directly to the PDF data 
structures.     That is really your problem - you are too high ;).

         It contains references to them - yes.  BUT you ALSO need to 
setup the /Annots array on each page as well...

         Add it to the /Page dictionary of that Page.
