Yes, we should start charging for iText...

I made 2 examples that explain different principles.

In the first example, I create a mask as done in chapter 5
of the book. I use an image with one component and 2 bits per
component as mask. It's rough, but nothing keeps you from using
4 or 8 bpc and augmenting the resolution:

In the second example I adapted your solution that would
perfectly fit in chapter 11. First the clipped image is added,
then cover it's covered with transparent template:
