By writing very low level code that analyzes the PDF structure and then adds
the necessary tags. I would give yourself at least 12 months of development
time, especially on the text/structural heuristics.

setTagged() will set the flag, but you're responsible to add the actual
tags (see appendix F of the 1st edition of "iText in Action").
