That's extremely difficult.

Not just with iText; it's extremely difficult in general.
You want to add information that isn't there.
How are you going to do that?
That would involve artificial intelligence!

I've tried this once using Acrobat, and even Acrobat made mistakes!
For instance, I had a list with names.

A. Geldof, JM. Mathijsen, P. Deschrijver,
J. Jannes, T. Vandewege,...

And Acrobat interpreted this list of names as a list with bullets
"A." and "J." ;-)

In other words: you've been posting several question in the last
few days that indicate that you have accepted an assignment that
is close to impossible (with or without iText).
