If you take a look at the output of the example site you refer to you can see that the producer is a word processor, in my case "Microsoft® Word 2010"; thus, most likely that application first tries to import the PDF document contents including all formatting information into an intermediate format (in my case MS Word), translate there and then export back to PDF. And the result is not too convincing, even given that the PDF->PDF translation is described to be in an alpha state. Similar (unfortunately also similarly limitted) results might be archieved following Bruno's example references.
