No sample PDF is provided and you're not telling us if you're using iText 5.4.0 or an older version.
I fear you won't get any useful suggestion.

By the way: maybe the file opens in Acrobat with no errors, but what does Preflight say? Preflight is functionality that is present in Acrobat and that allows you to check for syntax errors in a PDF. Sometimes a file looks OK in Acrobat, but Preflight informs you that the file contains syntax errors.
