So it is not specific to iTextsharp. Something happened when we upgraded Visual Studio to 2012 (from 2008) even though we were targeting the same .NET version. Apparently, something changed. We wound up choosing the same option as the blog article. That is, modifying the permissions on the temporary ASP.Net folder to allow full access by the impersonation user.
