I just want to say that you are not alone encountering this. (--log-status saying that the card is in radio mode, and then setting the frequency way wrong)
I have a local copy of ivtv-tune (from svn trunk) that has given the impression to work better than the one I got with my 0.6.1 packages, but I haven't really understood in what way it differs. To get things working when this happens, I just rmmod ivtv and then modprobe ivtv, then use this "good" ivtv-tune, and it usually works.
