Exactly what I had and the solution for me was to inbetween loading ivtv and ivtv-fb to do a dd if=/dev/video0 of=/dev/video16 bs=64k I believe John Harvey posted a patch to cure this a few days ago. I posted the little script I used in the "RE: [mythtv-users] Automatic startup with tv-out - solved" thread.
