Yeah, I think someone on the mythtv-users list mentioned that when using the TVout of the 350, there's no (easy) way to alter the audio stream realtime, since it just dumps the mpg data to the card for decoding and audio processing.
I guess one way around it would be to "process" the mpeg prior to playing back at the faster speed, with corrected audio. In essence, a transcode type deal, and then play back the new transcoded copy which is at the faster rate/fixed audio. Can't really think of any other way to do it with hardware decoding, though, I don't know the ivtv driver very well... Maybe there's some way to affect the audio decoding pitch?
