I'm running a D492 PVR-500 along with 2x PVR-250's and I live in NTSC land. I'm using FC3, 2.6.10-1.766_FC3 . IVTV version is ivtv-0.3.2w. modprobe.conf looks like this:
~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~
# ivtv
alias char-major-81-0 ivtv
alias char-major-81-1 ivtv
alias char-major-81-2 ivtv
alias char-major-81-3 ivtv
install ivtv /sbin/modprobe --ignore-install ivtv && /usr/local/bin/ivtv-settings
alias tveeprom tveeprom-ivtv
alias msp3400 msp3400-ivtv
alias tuner tuner-ivtv
options saa7127 i2c_enable=-1,-1,-1,-1
options cx25840 i2c_enable=1,1 no_black_magic=1
options ivtv ivtv_debug=1 tuner=2,2,57,57
~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~

I rename (mv), in /lib/modules/2.6.10-1.766/ivtv, by hand, post installation (ie., after make install):

msp3400.ko -> msp3400-ivtv.ko
tuner.ko -> tuner-ivtv.ko
tveeprom.ko -> tveeprom-itvt.ko

I do that last step, stupid as it seems, to avoid module ambiguity problems.

Order on tuner settings may very depending on which slots your cards are installed on. lspci is helpful here.

With that, everything works quite well for me. I haven't had any luck getting IVTV versions much newer than ivtv-0.3.2w to work properly for me though.

Hi!

Well, some of you may remember I was having problems with my new
PVR-500 MCE a few months ago. The revision is the D492 and the tuner
type is 57. I've attached that part of the DMESG at the bottom. I
always get the static that fades to black and darkish red... I've seen
several other people have this problem. I'm using the latest drivers:
0.3.4v
