Where do you live, and what's your local flavour of PAL?

I think you have the same problem as me; however, what follows is wild
speculation :) The FM1216ME (MK3) / FQ1216ME_MK3 tuner uses what I
believe is an incompatible PAL setting to previous Philips tuners, so
you're getting the default (whichever that is). Notice on that Wikipedia
page how the sound offset for PAL varies by up to 1 MHz depending on the
sub-type.

I notice you're using the "type=" override for your card; with the
latest driver, is the card correctly detected (i.e. listed in
tveeprom.c)?
