Does dmesg show any ivtv errors when this happens?

If you use the defaults for stream_type, bitrate and dnr_mode etc., do the 
same problems occur?

Would you be willing to try to determine when this problem was introduced? It 
could be time consuming but it may be the only way to find out where it went 
wrong.
