After more instigation and thinking I believe changing channels or the osd overlay aren't the issues.  After a few seconds of live tv the display goes back to the myth menuno matter what.  At first I was assuming it was me trying to change the channel.  I swapped the 0.4.0 and 0.4.1 drivers back and forth a few times to kick the tires.  Recording video from all cards on different channels works fine, as does watching recorded video.  The problem is probably in mythtv-frontend itself with piping live tv.  Hopefully the mythtv dev people will look into this issue.  For now everything works fine with the ivtv 0.4.0 drivers.
